forgot to even update current index/iterator in melody/symphony -_-
turned into a major rework, mainly to better detect transitions when
sequencing, but somehow also ended up making advancing on zero duration
illegal.

Ratio no longer default to float, too specific.
Total duration moved up to be the first member, as it's most essential.
Tests updated accordingly with some minor improvements.
